Social Worker, LCSW or LICSW
Location: Allston, MA 02134
Duration: 13 weeks
Shift: 8am-4:30pm
Pay rate: $63/hr. on w2 with benefits
Weekly expected hours: 40
Start date ASAP. Assess patients and families and provides intervention and treatment to support patients. Address psychosocial needs and patterns of adjustment throughout the hospitalization and collaborates with the multidisciplinary team to formulate treatment plans to maximize emotional, social and physical well-being. Provide information and education regarding health care and community resources. Completes MDS in conjunction with interdisciplinary team. Must hold current LCSW or LICSW license in MA. Skilled nursing facility or rehab experience preferred.
