Job Position Title: Home Health RN Intake Nurse
Reports To: Client Intake Coordinator/Agency Administrator
Employment Type: Per Diem/Retainer
Overview:
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home healthcare team as an Intake Nurse. This role is pivotal in ensuring a smooth and professional onboarding process for clients entering our agency. The Intake Nurse will assess clients’ needs, coordinate care plans, and collaborate with healthcare professionals to ensure the highest level of service and compliance with Medicaid and GAPP standards.
Responsibilities
- Conduct comprehensive assessments of patients' health status and needs in a home setting.
- Develop, implement, and evaluate individualized nursing care plans based on patient assessments.
- Monitor patients' progress and adjust care plans as necessary to achieve optimal health outcomes.
- Document all patient interactions, assessments, and interventions accurately in compliance with healthcare regulations
- Collaborate with other healthcare professionals to coordinate care and ensure continuity of services.
- Participate in team meetings to discuss patient progress and address any concerns.
What We Need from You:
We seek an RN with a current Georgia license, BLS certification, and a passion for home health care. You should have at least two years of experience, preferably with pediatric populations, and be well-versed in home care regulations.
Position Type:
- Contract/Retainer position ideal for RNs seeking supplemental income.
Compensation:
- Intake/Admissions: $70; Supervisory Visits: $50
