Summary

Creates, maintains, tracks, and updates all medical forms used by Akron Children’s Hospital. Responsible for coordinating form changes with departments, appropriate committees, regulatory groups (Legal, Risk, Compliance), forms vendor(s) and other responsible parties. Handle the design, testing and editing process of forms. Coordinate and manage the form approval process to achieve timely changes to forms and to comply with the Forms Policy. Assist with auditing the forms usage to ensure proper form version usage. Develop form standards.

Responsibilities

  • Oversees the medical paper form process and work to create and maintain an electronic forms process.
  • Act as a system administrator for form vendor software.
  • Serve as a knowledge resource liaison between HIM management, committee oversight, Information Services, Public Relations and the clinical areas that use the forms.
  • Oversees the maintenance of the library of the owner/originator, usage, and revisions of forms.
  • Ensure all forms created or revised have a current logo, space for patient identification, form title, form number, revision date, and barcode per the ACH Medical Form policy.
  • Resolve issues with form design and usage when issues arise.
  • Participates in committees related to forms, including Medical Documentation, Registration User Group, MyChart committees, etc.
  • Regularly review and update all forms to ensure all elements are up to date and in compliance with regulatory and legal requirements.
  • Perform regular audits on the documentation used and scanned into a chart to ensure proper form version usage is used.
  • Evaluate current process and forms to assess areas for process improvement.
  • Ensure all supporting policies and procedures reflect actual practice.

12. Assist with special projects related to medical documentation as needed.

Other Information

Technical Expertise

  • Experience with medical terminology is preferred.
  • Experience with Epic and Hyland OnBase/scanning software is preferred.
  • Proficiency in MS Office [Outlook, Excel, Word] or similar software is required, including spreadsheet design.
  • Proficiency with Adobe Acrobat Pro, Adobe Photoshop, Illustrator, PDF Editor preferred.
  • Experience working with all levels within an organization is preferred.
  • Experience in healthcare is preferred.
  • Excellent written, verbal, and interpersonal communication skills is required
  • Must be self-motivated, highly organized, detail-minded with strong commitment to accuracy, be exceptionally attentive to detail and spelling
  • Must be able to take direction and work independently while managing multiple priorities.

Education And Experience

  • Education: Associate degree preferred; or experience in health information management, graphic design or information systems in leu of degree.
  • Certification: none are required
  • Years of experience: Preferred 1-3 years of experience designing forms
  • Years of experience supervising: none needed
