Summary
The Patient Educator collaborates with patient/families, patient's school and/or homebound tutor, and other healthcare professionals involved with the patient's care to assess learning needs.
Responsibilities
- Collaborates with patients/families, patient’s school and/or homebound tutor, and other healthcare professionals involved with the patient’s care to assess learning needs.
- Plans and develops appropriate educational services for Hematology/Oncology patients in the inpatient unit and outpatient clinic setting, incorporating pertinent information from patient’s educational documents such as 504 plans and Individualized Education Plans.
- Provides educational instruction in individual and/or group settings using age appropriate interventions to meet the specific needs of each patient.
- Collaborates with Oncology Outreach Educator regarding identified learning difficulties or school concerns and school re-entry needs for patients served.
- Maintain open and ongoing communication with home school/homebound tutor regarding patient educational progress.
- Uses effective communication and collaboration to assess and promote patient’s educational goals
- Plans and implements appropriate educational services for Hematology/Oncology patients in the inpatient and outpatient setting
- Obtain and review pertinent educational documents such as 504 plans and IEPs and provide input into educational planning process
- Maintains documentation of communication and educational services provided
- Participates as an integral member of multi-disciplinary team
- Demonstrates the knowledge and skills necessary to provide care for the physical, psych/ social, educational, and safety needs of the patients served regardless of age.
- Other duties as required.
- Experience to make appropriate recommendations and interventions for patients' educational needs is required.
- Experience working with all levels within an organization is required.
- Experience in healthcare is preferred.
- Proficiency in MS Office [Outlook, Excel, Word] or similar software is required.
- Education: Bachelor’s Degree in Education is required.
- Certification: Valid State of Ohio teaching certificate [K-12] is required. BLS certification is required
- Years of relevant experience: 3 to 5 years is required
- Years of experience supervising: None.
