Dept/Program: Workforce Development/Recidivism Program
Reports To: Senior Program Manager, Workforce Development
Pay Rate: $18.00 - $25.00 (Depending on level of experience)
Classification: Regular/Full-time/Non-Exempt

Job Summary: Transition Specialist will build relationships with Participants and guide them as they work through their goals by planning and organizing action items for next steps. They will provide their Participants with access to supportive services and barrier removal plans. Transition Specialist will be measured on their Participants’ successful completion of the program, via goal progression and program compliance.
Under supervision and in accordance with policies, procedures and regulations, Transition Specialist will work with adult participants (18 years). Transition Specialist will ensure the delivery of quality services, which includes risk/needs assessment, individualized success plans, implement strategies that promote success, tracking of participant achievement and outcomes, referrals, and program exit.
Job Duties: Responsibilities include:
· Coaching Participants to establish goals and guide them through goal progression.
· Referral to additional services and programs based on participant needs and assessment data.
· Identify community resources and build community partnerships of services to eliminate participant barriers.
· Enter participants data into case management systems.
· Keep the Senior Program Manager informed of any updates or added supportive services, barrier removal, or collaborative efforts that are needed beyond the established Success Plan.
· Gather information on participants in training and offer feedback and suggestions for programmatic improvements.
· Other related duties as assigned.

Qualifications: Four-year degree preferred or minimum 2 years case management experience working with at-risk population, experience in workforce development preferred; Microsoft Office proficient; possession of a valid State of Ohio Driver’s License and transportation, as position requires travel within the State of Ohio with mostly local travel; fingerprint and background checks conducted.
Competencies: Excellent written and verbal communication skills; ability to articulate case plans to program participants and professionals; propensity for compassion and ability to relate to clients with various needs; ability to motivate clients to follow case plans; ability to build strong relationships with diverse individuals and groups; ability to be authentic and approachable and can garner trust; ability to remain focused, results-oriented and decisive; excellent organizational and recordkeeping skills, as well as the ability to manage multiple cases; sound critical thinking and problem-solving skills; ability to analyze feedback and determine next steps; strong computer skills to manage case records.
