Job Overview
We are seeking a dedicated Home Care Nurse to join our team. The ideal candidate will provide compassionate and high-quality care to patients in their homes, ensuring their well-being and comfort, under the orders of the primary care physician.

Duties
-Providing patient care according to the Nurse Pactice Act and agency policy which includes systematic data gathering, assessment, and appropriate nursing judgement and evaluation of human responses to actual or potential health problems.

-Complete and submit all patient care documentation and departmental documentation in a timely manner.

-Communicate professionally and appropriately with all internal and external clients, ensuring that all information is shared accurately, timely and appropriately.

-Complete on call assignments according to agency and program policy, providing 24 hour coverage.

Requirements

Bachelor's degree from an accredited* college or university in Nursing and one year of professional nursing experience in home care.

OR

Associate's degree from an accredited* college or university in Nursing or diploma in Nursing and three years of professional nursing experience including one year of experience in home care.

Pay: $52,761.60 - $93,096.00 per year
