Summary
Responsible for providing physicians a comprehensive medical history of the patient by confirming relevant medical information and entering into the electronic record. Responsible for updating and maintaining medical record for each patient in the clinic.
Essential Duties and Responsibilities
- All other duties as assigned.
- Updates and maintains medical record for each patient in the clinic
- Follow up on medical records as needed based on physician visits
- Enter all providers and collect direct messaging addresses for any provider with no DM address
- Documents previous treatment histories and ongoing treatment events
- Enters annual patient updates.
- Reverifies patient paperwork.
- Sends “Care Space” invites on dates of patient visit.
- Administer and sends out cancer questionnaires.
- Enter Annual Patient Updates, checking and monitoring closing referral loops and medical records from referrals
- Sends portal invites daily
- Establish and maintain effective working relationship with coworkers, supervisors, the general public, and referring offices.
- Performs various clerical and administrative functions as needed.
- All other duties as assigned.
- Excellent written and verbal communication skills
- Maintains a regular and consistent professional attendance, punctuality, personal appearance, and adherence to relevant health and safety procedures.
Education and Experience
- High School diploma or equivalent.
- Must have a well-developed understanding up medical terminology including medical diagnosis codes.
- Must be able to operate general office equipment and programs including, but not limited to, MS Office Suite, Electronic Medical Records (EMR) program, computer, copier and fax.
- Minimum one (1) year experience in a medical data entry role.
- Experience in Oncology is highly preferred.
