What are the responsibilities and job description for the Public Health Nurse III- Lead Nurse Coordinator and Supervisor for Care Management (CMHRP) position at Alamance County Government?
This position serves as lead nurse coordinator and supervisor for the CMHRP program and provides direct care management services for eligible pregnant women residing in Alamance County. This position supervises two social worker CMHRP positions, which includes annual performance appraisals and work assignments with oversight from care management nursing supervisor.
CMHRP services are provided for eligible pregnant Medicaid recipients who are determined to be at risk for poor birth outcomes. This service includes assessment, planning, facilitation, care coordination, evaluation and patient advocacy to address health care and preventative service needs of pregnant and postpartum women. Care management focuses on coordination of the patients’ medical, nutritional, educational, social and emotional needs with private and community resources to promote healthy pregnancy outcomes. Services are provided within the community, at local physician practices and within the patient homes. Patient home visits and monthly face to face patient services are required to meet program measures. This position functions as a part of a multidisciplinary care management team. As the nurse on this team, this position may be assigned more medically complex patients and, as needed, will serve as a nursing resource to other non-nurse team members. As part of the team, the nurse must be familiar with and aim to achieve the programmatic goals set forth by the prepaid health plans and North Carolina Medicaid.
As a part of routine duties, this position is also responsible and required to utilize multiple electronic medical records and state electronic systems, collect, review, assemble and report program data to supervisor providing input on staff and program performance, maintain and update program policy and procedures, and assist supervisor with orientation and training of new staff as well as assisting student learners.

ESSENTIAL KNOWLEDGE, SKILLS, AND ABILITIES:

ESSENTIAL PHYSICAL ABILITIES: Medium type work (exerting up to 50 lbs. of force) with physical abilities that include: stooping, kneeling, crouching, reaching, standing for long periods of time, walking, pushing, pulling, lifting, fingering, grasping, talking and hearing. Subject to repetitive motion of the wrist, hands, and/or fingers. Close visual acuity is required to perform duties at a computer terminal and using measuring devices. Subject to inside environmental conditions such as temperature changes. May be exposed to blood or body fluids in performance of job duties.
MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS:
Graduation from a four-year college or university with a B.S. in Nursing which includes a Public Health Nursing rotation and two years of Public Health nursing experience; or a Master’s in Public Health and graduation from a school of professional nursing and two years of professional nursing experience; or graduation from a school of professional nursing and three years of professional nursing experience including two years of Public Health nursing experience; or an equivalent combination of training and experience.
NECESSARY SPECIAL QUALIFICATIONS: 1: A current license to practice as a registered nurse in North Carolina by the NC Board of Nursing. 2: Valid N.C. driver's license and have access to a personal vehicle. License to practice as a registered nurse in NC (if nurse applicant).
PREFERRED QUALIFICATIONS:Knowledge and experience with Maternal-Infant Health/Pediatrics preferred. Fluent in both Spanish and English both oral and written preferred.
