Overview of the Position
We are currently recruiting for a Full Time Family or Adult Medicine Physician to join our team at Hayward Wellness Center, part of Alameda Health System. As a provider of care to all patients, regardless of their ability to pay for services, we provide quality, accessible care to our community's most vulnerable patients.
Responsibilities
- Delivers high-quality, patient-centered care to patients of all ages, from pediatrics to geriatrics.
- Works collaboratively with other healthcare professionals to ensure seamless coordination of care.
- Develops and implements individualized treatment plans that take into account each patient's unique needs and goals.
- Maintains accurate and up-to-date records of patient care, including lab results, medication lists, and progress notes.
- Provides education and support to patients and their families, empowering them to take an active role in their healthcare.
- Participates in quality improvement initiatives and ongoing education to stay current with best practices and emerging trends in medicine.
Requirements
- Must have completion of an approved residency in family medicine or adult medicine.
- Possession of a current valid license to practice medicine in the state of California, AND Board Certification or be Board Eligible in area of specialty.
- Board Certification must be completed within three years of the date of hire.
- Possession of Basic Life Support Certification issued by the American Heart Association and a valid Drug Enforcement Agency (DEA) Number.
