Job Summary:
We are seeking a highly motivated and detail-oriented Case Manager RN to join our team. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
Key Responsibilities:
- Develop and implement individualized care plans to meet patients' unique needs and goals.
- Collaborate with interdisciplinary teams to ensure comprehensive care coordination.
- Conduct thorough discharge planning assessments and coordinate post-discharge care arrangements.
- Provide education and support to patients and families to promote self-management and independence.
- Stay up-to-date with current resources and services available in the community and make referrals as needed.
Requirements:
- Bachelor's degree in a related field; Master's degree preferred.
- Minimum one year of experience in acute care facility and/or utilization review training and/or healthcare experience.
- Valid license to practice as a Registered Nurse in the State of California.
- BLS - Basic Life Support Certification issued by AHA-American Heart Association.
