Primary Responsibilities 

Performs highly complex (senior-level) case management work in fast-paced environment to meet the needs of individuals with intellectual and developmental disabilities targeting those at risk or with comorbid substance use disorders; Work involves reviewing and tracking case management activities, and assisting in developing program goals, objectives, and procedures; Assigns and/or supervises the work of others; Works under limited supervision, with considerable latitude for the use of initiative and independent judgment.

 

Tasks Performed

Oversees the development and implementation of clients’ service and treatment plans; Provides technical assistance to other staff and support team members of client; Reviews clients referred to case management and tracks progress through the case management intake and screening process; Reviews all case documentation for accuracy and timely submission; Prepares reports on service provider and program activities; Identifies problem areas, service gaps, and areas that are barriers to services for clients; Documents case records, and prepares and reviews reports and evaluations; Performs contract monitoring activities for the unit; Coordinates the development and implementation of clients’ service and treatment plans; Coordinates transition planning to ensure continuity of  care and management of risk factors; Assists in the development of policies and procedures; Assists with program monitoring and evaluation, including production targets and performance metrics; Prepare reports for funders that contain aggregate data collected; Assists with intra-agency or interagency services coordination and referral problems; Assigns and/or supervises the work of others; Conducts performance reviews; Make recommendations regarding hiring, promotion, and termination decisions; Disseminates information to the team and forwards information to the manager in a timely manner; Works closely with team members to resolve customer problems; Develops the team to ensure their consistent delivery of required performance measures. Conducts community outreach and training; Ensure delivery of services for Project of Prevention & Treatment of Substance Use Disorder (SUD) in IDD Population.

Essential Functions 

Prefer knowledge of Intellectual and Developmental Disabilities, Autism, Related Conditions Servicers and funding sources; Knowledge of community resources, of case management delivery systems, principles, objectives, standards, methods, and of program policies and procedures; Skill in overseeing, developing, implementing, and monitoring service and treatment plans; Ability to assess clients’ needs, to oversee and coordinate clients’ services, to monitor program effectiveness, to communicate effectively, and to assign and/or supervise the work of others; Ability to apply motivational interviewing techniques when speaking with clients and community; Should have background in SUD prevention, education, and treatment interventions; Ability to train others; Knowledge of rehabilitation programs for individuals with developmental disabilities; Skills in the use of a computer and applicable software; Ability to prepare and maintain records; Extensive travel within Bexar County required; Must have reliable transportation; Ability to lift 10 – 15 pounds is required; Adequate health and physical mobility to get up and down off the floors as required First/CPR provision and application of SAMA techniques; Perform other job related duties as assigned.

 

Desired Degree/Job Experience

Prefer Master’s  degree in Psychology, Counseling, Social Work or related field; A license in one of the following LPC, LPC-I, LMSW, LCSW, etc. Must have two (2) years’ experience in case management or social services work and two years of paid or unpaid experience with individuals with intellectual or developmental disabilities is required, as defined in Texas Administrative Code, Title 26, and Rule 331.17; Prefer ability to meet specific requirements of Title 42, CFR, Section 483.430. 

 

 

Other Qualifications

Prefer bilingual fluency in both English and Spanish.
