Palliative Care Nurse Practitioner
Alamo Hospice and Palliative Care is seeking a compassionate Palliative Care Nurse Practitioner to support San Antonio and surrounding counties.
Job Summary
The ideal candidate will collaborate with the Palliative Care Medical Director and team members to deliver palliative care by providing treatment and follow-up for patients referred to palliative care. This role requires working independently, making clinical judgments, and interacting with patients, families, and healthcare professionals.
Duties and Responsibilities
- Work in collaboration with the Palliative Care Medical Director and other team members to deliver palliative care by providing treatment and follow-up for patients referred to palliative care.
- Care for patients in their home within a 60-mile radius of our San Antonio, TX office.
- Provide regular visits to reassess the patient's needs, concerns, and update the plan of care.
- Initiate communication with the Primary Care physician, and other specialists/agencies involved in patient care.
- Prescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law.
- Order and interpret laboratory and other diagnostic tests within the scope of professional practice.
- Order treatments and durable medical equipment as indicated.
Requirements
- Graduate from an accredited registered nursing program.
- Licensed as a Nurse Practitioner in the state of practice.
- Minimum of two years of varied work experience as a professional nurse.
- Must possess current CPR Certification.
- Demonstrated skill in nursing practices and clinical judgments.
- Ability to make home visits and provide nursing care in the home environment.
- Ability to cope with family emotional stress and tolerance of individual lifestyles.
- Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
- Valid driver's license and proof of insurance is required.
