What are the responsibilities and job description for the Social Worker Per Diem position at Alaris Health at Kearny?
Alaris Health at Kearn, a 120 bed-skilled nursing Center in Kearny, New Jersey, seeks a social worker to plan and implement social service programs in the Center and coordinate discharge planning. Additional responsibilities include :
- planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families
- evaluating and being keenly aware of residents’ medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan
- coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families / responsible parties and the treatment team
- maintaining residents’ rights at all times while liaising between all parties involved
- coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents
- and above all, ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights
