Alaris Health at The Chateau – North & South Pavilion, a 251 bed-skilled nursing faciltiy in Rochelle Park, NJ, seeks a physical therapist assistant PTA to implement physical therapy programs and provide treatments to residents to facilitate increased independence and functioning under the supervision of a licensed physical therapist. Additional responsibilities include :
- implementing residents’ individualized treatment plans as defined by the primary physical therapist
- providing individualized physical therapy treatments that include modalities and exercises
- instructing residents, families and other caregivers in the skills and techniques of the physical therapy treatment program under the supervision of the physical therapist
- collaborating with other members of the rehabilitation team to develop programs and activities consistent with the needs and capabilities of each resident
- maintaining appropriate and timely resident and non-resident documentation for all residents treated
