ESSENTIAL FUNCTIONS:
- Basic understanding of coagulation, antithrombotic therapy and thrombogenesis.
- Basic understanding of Warfarin and other anticoagulants used in the outpatient setting.
- Performs patient interview and assess patient’s needs regarding upcoming visits and/or procedures and any current concerns with anticoagulant medications.
- Assessment skills to identify the signs and symptoms of emergent side effects of treatment, and when to refer to the pharmacist or primary cardiologist.
- Provide phone calls to patients to communicate medication dosages and next labs tests for homebound and nursing home patients on our remote monitoring service (Acelis).
- Compliance with Universal Precautions and hospital as well as clinical guidelines for blood specimen collection.
