Summary:
Under general direction and supervision of supervisor or director, or their designee, provides clinical social work services for ANMC Clients.
Responsibilities:
Serves as an entry level professional social worker. Performs psychosocial assessment and planning for patients with a variety of complex social service’s needs; diagnoses and treats psychosocial issues that present with healthcare and impacts the wellness of the individual. Provides social services plan to restore patient to optimum overall health within patient’s capacity and safe discharge. Collaborates with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors affecting patient’s health problem. Facilitates appropriate use of acute care resources.
Assists patient and family through individual or group conferences to understand medical recommendations and options to facilitate patients and families engaging in treatment planning and decision making. Develops discharge options and assists with placement. Utilizes community resources and support to assist patient in resuming life in community or to learn to live within limits of disability. Provides general direction to healthcare professionals engaged in patient program activities.
Participates in planning for improving health services by interpreting social factors pertinent to development of program. Prepares patient histories, service plans, and reports. Ensures qualitative, complete and accurate patient documentation. Records, analyzes and interprets statistical data. Effectively solves practical problems and presents information and responds to questions from managers, clients, customers, and the general public. Assesses quality improvement needs and recommends and participates in program development. Participates in workgroups, interdisciplinary rounds and hospital-wide committees.

MINIMUM EDUCATION QUALIFICATION
Master’s degree in social work from an accredited school of social work.
MINIMUM EXPERIENCE QUALIFICATION
Non-supervisory - Two (2) or more years of acute care medical social work experience.
PREFERRED EXPERIENCE QUALIFICATION
Experience in the Alaska Tribal Health System.
PREFERRED CERTIFICATION QUALIFICATION
Alaska License in Clinical Social Work (LCSW) preferred at time of hire.
ADDITIONAL REQUIREMENTS
· Alaska License in Clinical Social Work (LCSW) required within three (3) years of hire.
· Depending on the needs of the organization , some incumbents in this job class may be required to obtain additional certifications or training in one or more specialty areas.
