As a Social Care Navigator, you will collaborate with individuals and families to identify health-related social needs (HRSN), link them to available community resources, and address barriers to accessing services, fostering stability and empowerment.
ACAP is hiring a full-time Social Care Navigator to join our team at our main office in Albany, New York.

Pay starts at $22.49 per hour; Monday – Friday; 37.5 hours per week
Qualifications :
A high school diploma or equivalent, along with a minimum of four years of experience in social work, case management, or care coordination, is required. An associate's degree in human services, social work, psychology, or a related field is preferred and can substitute for two years of the required experience. Intermediate PC skills including database entry, word processing, and spreadsheet skills and experience with social services or benefits administration is required. Bi- or Multi-lingual skills are a plus.
Responsibilities :
- Address enrollment and eligibility phone calls, emails, and in-person appointments, answering questions, resolving problems, and facilitating access to care for eligible community members;
- Serve as the primary point of contact for community members, external service providers, ACAP program staff, and other stakeholders regarding enrollment, eligibility, and referrals to health-related social activities and needs;
- Act as a liaison between community resources and community members, following-up and advocating when needed to ensure timely responses for referrals;
- Monitor internal and external referrals several times a day, engaging community members to provide prompt assistance and follow-up;
- Collect and maintain up-to-date documentation of enrollment, eligibility, and other programmatic data in multiple databases.
