This vacancy is for a Public Health Aide II (Family Support worker) in the Healthy Families Home Visiting Program.
This employee provides assistance and direction to families with children in need of a wide range of special services for disabilities. The incumbent in this class is responsible for identifying, assessing and coordinating needed services as shown by case management plans on individual children and families. The incumbent in this class is also responsible for developing an understanding among local providers of health and social support services and the population regarding the need for accessible and acceptable primary health and social services, encouraging community organizations to needed services and broadening the understanding and knowledge of various health and human services providers regarding the population’s perceived barriers to care. Work may be performed during evening or weekend hours. Does related work as required.
Principal Job Duties:
The duties of this position would include, but are not be limited to managing a caseload of assigned families; establishing positive, productive, and professional relationships with client families; promoting parent-child interaction, family functioning, and development of self-sufficiency; assessing family functioning and making appropriate referrals for additional resources and services as needed; serving as a liaison between the family and other service providers.
The candidate for this position should possess strong organizational and time management skills, effective communication skills, strong writing skills, work well with diverse populations, and possess a working knowledge of Albany County community resources. Additionally, have the ability to collaborate and coordinate services and work with various providers to assist families.
Minimum Qualifications (either):
A. Graduation from an accredited college or university with at least an Associate’s degree in Sociology, Health Services, Human Services, Psychology, or a closely related field and six (6) months of experience working with or providing services to culturally diverse children and families; OR,
B. Possession of a high school diploma or general equivalency diploma and one (1) year of experience working with or providing services to culturally diverse children and families.
Special Requirement:
1. Valid NYS driver's license.
2. Ability to work flexible hours including evening.
3. Successful clearance from the State Central Registry in the New York State Office of Children and Family Services (Background investigation is conducted by the appointing authority).
