About the Opportunity:

Main Responsibilities:
- Deliver comprehensive nursing care to patients across the life span, utilizing evidence-based practices and individualized care plans.
- Collaborate with interdisciplinary teams to coordinate care, promote health education, and optimize patient outcomes.
- Provide age-appropriate direct patient care, including diagnostic procedures, medication administration, and assistance with physician orders.
- Participate in telephone triage, coordination of care, and documentation to ensure seamless patient care.
- Assist with community outreach, patient education, and navigation through the healthcare system.
- Maintain accurate and thorough medical records, ensuring compliance with regulatory requirements.
Requirements:
- Associate's degree from a state-accredited Registered Nursing program.
- NYS current RN licensure to practice professional nursing.
- Certification in specialty or Ambulatory setting preferred.
- Familiarity with electronic health records and computer software.
- Working knowledge of MS Word and Excel.
