Job Description:
The Registered Professional Nurse in the ambulatory setting provides care across the life span to individuals, families, caregivers, groups, populations, and communities. This is accomplished during face-to-face encounters or through a variety of telecommunication strategies, often resulting in the establishment of long-term patient care relationships.
During each encounter, the registered nurse focuses on patient safety and the quality of nursing care by applying appropriate nursing interventions, such as identifying and clarifying patient needs, performing procedures, conducting health education, promoting patient advocacy, coordinating nursing and other health services, assisting the patient to navigate the healthcare system, and evaluating patient outcomes.
Ambulatory care registered nurses, acting as partners and advisers within the interdisciplinary team, assist and support patients and families to optimally manage their healthcare, respecting their culture and values, individual needs, health goals, and treatment preferences.
Essential Duties and Responsibilities:
- Completes organized and quality documentation as required by the department.
- Facilitates the utilization of resources to meet patient outcomes (case management).
- Analyzes patient care needs and utilizes a systematic approach to delivery of care.
- Provides age-appropriate direct patient care from the preparation of patients through the end of the office visit and all tasks in between, including diagnostics, venipuncture, administration of medications, and assistance of physicians as needed.
- Performs telephone triage, including coordination of care and documentation.
- Schedules tests and procedures as indicated within the division and obtains prior authorizations as needed.
- Works with physicians and other resources to develop teaching tools for families and patients.
- Performs community outreach as indicated.
- Works on studies with Principal Investigator as indicated within the department.
- Provides patient and family education, taking into consideration cultural, religious, language, motivational, physical, emotional, and cognitive factors into account in the teaching plan.
- Works with other departments inside and outside of AMC to coordinate multidisciplinary care.
- Ensures that medical equipment is functioning properly and knows the process to manage failure as needed arises.
- Participates in the patient throughput process in the clinic as indicated and needed.
- Orients new staff.
- Works with division physicians, administrative staff, and Nurse Manager to assure compliance with regulatory conditions and policies and procedures.
- Actively participates in the survey process regarding accreditation.
- Utilizes the scheduling system lookup to ensure appropriate follow-up has been scheduled.
- Locates regulations and policies and procedures online and knows how to apply them.
- Utilizes educational resources on the Intranet for self-education and education of other staff and families.
- Delegates daily tasks to appropriate clinical staff and ensures completion of tasks.
- Collaborates with administrative staff and physicians on the Quality Improvement Team to address clinical and non-clinical issues within the division.
- Maintains compliance with regulatory agencies.
- Reviews and triages incoming test results as indicated within the division.
- Documents properly, giving age-appropriate care, gathering data correctly, and facilitating care in a customer-friendly manner.
- Maintains compliance with AMC standards for electronic system use.
- Maintains compliance with AMC Code of Conduct.
- Maintains a safe environment within the clinic and working office space.
- Realizes their role in Environment of Care Surveys.
Minimum Requirements:
- Associate's degree from a state-accredited Registered Nursing program is required.
- NYS current RN licensure to practice professional nursing is required.
- Current CPR certification is required.
- Certification in specialty, Ambulatory setting preferred.
- OB experience preferred but not required.
Skills Required for the Position:
- Excellent customer service and communication skills.
- Ability to effectively present information and respond to questions from physicians, patients, and their family members or other employees within the work setting. Familiar with computer software.
- Effective Written and Verbal Communication Skills.
- Working Knowledge of MS Word and Excel.
- Experience with electronic health record preferred.
- Ability to give medications via all routes and ability to calculate medication doses.
- Proficiency in the operation of standard office equipment and clinical equipment.
- Must be a team player and multi-task within a fast-paced clinical environment.
