Skilled Nursing Facility is looking to hire an RN Admissions Nurse in Albany, NY.
Responsibilities:
- Conduct initial assessments for new residents and hospital returns, initiating care plans accordingly.
- Assist in overseeing the Nursing Department under the guidance of the Director of Nursing.
- Review PRI assessments and collaborate with the interdisciplinary team to manage the admission process.
- Audit medical records to ensure compliance with facility standards.
- Participate in discharge planning as needed.
- Update and revise residents’ care plans as their conditions change.
- Maintain thorough and accurate documentation per facility policies and procedures.
- Complete MDS, care plans, and other required documentation.
- Ensure all resident information remains confidential in compliance with HIPAA and company policies.
- Supervise and oversee resident care as necessary.
Qualifications:
- Active and unrestricted NYS Registered Nurse (RN) license.
- Previous experience as an admissions nurse in a long-term care setting.
- Strong clinical assessment abilities.
- Capable of making independent, sound decisions.
- Strong leadership and supervisory skills.
- Proficiency in MS Office (Excel, Word, Outlook) and other computer applications.
- Long-term care experience is preferred.
- Excellent communication and interpersonal skills.
