Positions Responsibilities:
This position will assist with The Housing Opportunities for People with AIDS(HOPWA) program. The primary goal of the HOPWA Program is to increase access to and ensure the provision of safe, decent, and affordable housing and housing-related services to persons living with HIV infection in North Carolina and their families, and to ensure that all recipients living with HIV infection are receiving medical care. The counties designated as Regions 9 Network of Care and served by Albemarle Regional Health Services are Beaufort, Bertie, Chowan, Dare, Gates, Hertford, Hyde, Pamlico, Pasquotank, Perquimans, Tyrrell and Washinton. This position will serve as the HOPWA Environmental Technician working directly with clients in the respective counties, to ensure stable, safe housing with functioning utilities are maintained. Focus will be on housing support services, coordinating with the Healthy Opportunities Intervention Specialist on housing support and safety. This position will accept program referrals, ensure timely and efficient patient coordination and reporting, provide community education and recruitment for program participation, and ensure all Housing for Urban Development (HUD) federal housing program requirements are met as outlined by the state Agreement Addendum for the HOPWA program.
Key Duties:
· Receive referrals related to housing support, coordinating with the HOPWA guidelines s needed.
· Follow up with clients within 72 hours of referral receipt.
· Schedule and conduct necessary housing environmental safety inspections.
· Attend Regional Network quarterly meetings
· Provide necessary case management services and linkages to resources and care.
· Document all communication and actions taken in CAREWare and CureMD electronic medical record as needed.
· Coordinate with Infectious Disease Clinical teams within Regions 9 and 10 Networks and programs as needed.
· Ensure invoicing and billing processes are coordinated through the landlords, mortgage companies and utility agency with approval from HOPWA Supervisor accordingly
Knowledge, Skills, and Abilities:
· Working knowledge of sampling procedures and the operation and maintenance of monitoring equipment.
· General knowledge of and ability to prepare certain chemical solutions for conducting routine tests.
· Ability to read maps and locate assigned stations.
· Ability to perform minor repairs on equipment and vehicles.
· Ability to perform limited calculations.
· Ability to make observations of physical conditions at assigned sites, determining whether the housing unit is eligible for financial assistance based on HUD guidelines.
Minimum Training and Experience:
High school diploma and one year of experience which provides the required knowledge and abilities to perform the work; or an equivalent combination of training and experience.
