Job Description

Job Title : Nurse Practitioner

Job Specialty : Geriatric Medicine

Job Duration : 6-month assignment with the possibility to extend

Shift : Days, Monday - Friday, 20 hours per week, part-time

Guaranteed Hours : Not specified

Experience : Minimum five years of experience preferred, ideally in long-term care

License : Active state license required

Certifications :

Board Certified by ANCC or AANP required
BLS, CDS, DEA required
Certification as a Nurse Practitioner with prescriptive authority required

Must-Have :

Master's degree in Nursing

Minimum of one year working with the frail or elderly population strongly preferred

Valid driver's license, proof of insurance, and means of transportation

Job Description :

Provide primary medical coverage in collaboration with physicians as needed

Perform initial, semi-annual, and annual assessments, and additional assessments of participants as needed

Implement appropriate care plans for participants

Participate in discharge planning for acute and long-term placement

Evaluate and treat participants during acute illnesses

Prescribe medications, therapies, and other treatments prudently

Consult with the Primary Care Physician for specialist referrals as indicated

Conduct nursing home or home visits when needed

Collaborate with the Medical Director and Clinic Supervisor to formulate clinical policies, procedures, and standards of care

Assist with the development of standards of care, ongoing monitoring, and evaluation of patient care practice and service delivery

Provide guidance and training to staff regarding medical and quality assurance issues

Advise the Medical Director and / or Primary Care Provider on enhancing accountability of services to participants and referral sources

Act as a resource during intake of new participants and contribute to the day-to-day operation of the center programs
