We are looking for an experienced nurse practitioner to serve our patients in Northwest Georgia and Southeast Tennessee. The ideal candidate will be licensed in both GA & TN.
Our team treats elderly and other adults suffering from illness, injury, chronic conditions, and other complex health issues in the safety, privacy, and comfort of their homes.
We are growing and seeking a Visiting Nurse Practitioner to share the same passion for excellence to deliver high quality and compassionate primary care medicine.
The Visiting Nurse Practitioner delivers primary health care and patient care management in a residential setting.
Essential Duties and Responsibilities
- Takes history, examines, determines diagnoses, and treats scheduled patients
- Provides written documentation of patient's visit, per NCQA standards
- Takes patient vital signs, as necessary, if Medical Assistant is unable to obtain
- Instructs patients/caregivers on proper medication and treatment regimens
- Works with Patient Care Coordinator to assure orders and referrals are properly/timely placed
- Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis and treatment
- Maintains patient confidentiality
- Attends required meetings and in-services and participates in committees, as requested
- Participates in professional development activities and maintains professional licenses and affiliations
- Performs related work as required
REQUIRED Knowledge, Skills, and Experience
- Certified Nurse Practitioner
- Master's (Preferred)
- RN (Preferred)
- Minimum of 2 years of Nurse Practitioner experience
- Active DEA license or seeking to obtain license
- Preferred experience in Geriatrics or complex patients
- Preferred EMR experience
- Visiting NP will be 'On the road'
