Reports to: Community Support Services Supervisor

FLSA Status: Exempt

Alder Health Services is seeking a full-time Medical Case Manager to provide services for HIV individuals accessing the agency's community support services. Successful candidates will exemplify the agency's patient-focused, strength-based approach to case management services and Alder Health Services’ commitment to providing high-quality and accessible programs that are dedicated to meeting the unique needs of the LGBTQ community, individuals living with HIV/AIDS, and those struggling with substance use disorder(s) within our communities.

Summary:

The position provides support and advances the mission and vision of Alder Health Services through the coordination of services and programs for HIV individuals including the coordination of medical care and working in collaboration with HIV specialist medical providers. The position assures the high-quality and patient-centered delivery of all collaborations between individuals in the community and Alder Health Services. Medical Case Managers recognize the unique needs of persons who are HIV and individuals who have traditionally been marginalized by the healthcare system. The position reports to the Support Services Supervisor.

Major Duties and Responsibilities:

· Provide strengths-based, client-centered & client-driven case management and support services.

· Provide services to clients in a variety of settings including but not limited to: Alder Health Services offices, medical appointments, social service appointments, client homes, and other establishments as mutually agreed upon, applicable, and appropriate.

· Schedule and conduct intake interviews with referred and/or assigned clients.

· Complete client needs assessments with all individuals served.

· Document services in CAREWare, an online Department of Health record system in a clear, concise, and complete manner.

· Develop, monitor, and update comprehensive, individualized service coordination plans in a SMART format for each client.

· Coordinate linkages to services, entitlement programs, and community resources necessary to address client identified health needs and barriers to care.

· Connect individuals to available and applicable services at Alder Health Services including medical case management, wellness groups, primary and mental health care, pharmacy services, health education and STD/STI testing, and client assistance programs.

· Participate in mandatory and voluntary professional development trainings.

· Remain current in all expectations required by the Pennsylvania Department of Health in conjunction with the Pennsylvania Case Management Project to maintain qualifications to provide HIV case management in the Commonwealth.

· Remain current on HIV treatment issues and community resources/agencies for referrals.

· Prepare documentation of services delivered, outcome measurements, and other desired reports as assigned and requested.

· Maintain a positive and professional work relationship with community partners and Alder Health Services staff.

· Ensure the highest level of confidentiality for all clients, client physical and digital records, and client related issues.

· Serve as an advocate for the needs of individuals impacted by HIV/AIDS and individuals who have traditionally been marginalized by the healthcare system.

· Ensure that clients are involved in all phases of case management practice to the greatest extent possible.

· Other case management duties assigned by the Community Support Services Supervisor.

Qualifications:

Language Ability:

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and case notes. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Math Ability:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.

Required Job Related Skills/Competencies:

1. Familiar with practices that are trauma informed, culturally sensitive, strength-based, and utilize person-first language.

2. High level of comfort working with diverse populations, specifically the LGBT community and individuals struggling with substance use disorder(s).

3. Ability to work collaboratively in a team environment dedicated to high quality client care, support, and service delivery.

4. Basic computer skills, including Microsoft Office, digital cloud system use, and ride-share programs.

5. Strong interpersonal skills.

6. Critical thinking, deductive reasoning and decision-making skills.

Education and Job Requirements:

  • Candidate will possess a bachelor’s degree in social work, psychology, sociology, or other related field OR a combination of Relevant education and experience.
  • Must possess a Pennsylvania Driver’s License.
  • One-year of experience working in the human services field is required.
  • Must possess reliable, personal transportation to be used throughout the workday. This position works throughout the community and requires a vehicle to complete the assigned job duties.
  • Ability to work independently and manage own schedule when working in the office and remotely.

Preferred Qualifications:

· Bilingual (Spanish/English) preferred.

· Previous experience working with diverse populations.

· Knowledge of HIV/AIDS and the NASW Code of Ethics.

· Experience creating service coordination plans.

· Familiarity with governmental assistance programs and community resources.

Pay: $48,000.00 - $55,000.00 per year
