As the Care Delivery Manager, you will play a key role in enhancing patient satisfaction and outcomes by :
Overseeing personalized care plan development and execution, ensuring a high quality of care.
Acting as the manager on-call to support patients and caregivers, handling documentation and grievances when needed.
Allocating staffing resources efficiently to ensure seamless site operations and team coverage.
Collaborating with clinical staff to maintain accurate assessments and uphold regulatory compliance.
Coordinating long-term care services as needed in various settings (nursing homes, rehab facilities).
Supporting the professional growth of team members through onboarding, training, and performance evaluations. Who We Are Looking For :
Experience : Bachelor of Science in Nursing (BSN) with a minimum of 2 years of experience in a clinical setting, or 5 years of relevant experience in healthcare, preferably in an administrative management role within home healthcare, long-term care, acute care, or similar environments.
Licensure : Current NY State RN license.
