Compensation : $99,000 to $113,000 Annually (Depending on Experience)

What You Will Do
As a Physical Therapist, you’ll be at the heart of patient care, providing therapy to help participants regain function, alleviate pain, and prevent future disabilities. You will play a key role in improving quality of life for individuals in a PACE program, including home visits and evaluations at various sites such as nursing homes and community settings.
Key Responsibilities :
Conduct comprehensive evaluations and reassessments, developing and implementing personalized treatment plans.
Deliver physical therapy treatments both in the clinic and in participants’ homes or other facilities.
Collaborate with interdisciplinary teams to create and adjust care plans that meet the needs of each participant.
Educate patients, families, and staff to promote functional independence and safety.
Maintain up-to-date documentation of participant care, treatment plans, progress, and statistical data.
Mentor and supervise Physical Therapy students, assistants, and volunteers.
Ensure patient safety and follow strict health and safety protocols.
Actively participate in care team meetings and training programs to enhance clinical and team effectiveness.
What We’re Looking For
To succeed in this role, you should have :
Education : Successful completion of a Physical Therapy program from an APTA-accredited university or equivalent.
Experience :
At least 1 year of experience as a Physical Therapist.
Experience working with elderly or frail populations (geriatric care experience preferred).
License : Active New York State licensure as a Registered Physical Therapist.
Additional Skills :
Knowledge of PACE regulations is preferred.
Proficiency in electronic medical records (EMRs) like Care Compass.
Bilingual skills are a plus, depending on participant demographics.
Strong communication and interpersonal skills to build rapport with patients and staff.
Ability to travel to different patient sites, homes, and clinics within the Brooklyn area.
