About the Role : As an RN Care Delivery Field Supervisor, you’ll lead and oversee clinical services delivered directly to patients’ homes, ensuring the highest standards of care. You’ll work collaboratively as part of a dedicated management team, bringing guidance and support to field nurses and home health aides, and help deliver person-centered, high-quality care to participants.
What is Offered :
Competitive Salary : Earn between $125,000-$130,000 annually, based on your experience and credentials.
Full-Time, Flexible Schedule : Work Monday through Friday, 8 : 30 AM - 5 : 30 PM, offering a stable work-life balance.
Prime Location : Conveniently based in Queens, NY.

Supervise and support a team of Community Health Nurses (CHNs), Personal Care Aides, and Home Health Aides.
Conduct joint visits and oversee care, ensuring compliance with individual care plans.
Collaborate with the Interdisciplinary Team (IDT) to create and implement customized care plans for each participant.
Provide hands-on education to participants and staff on best practices, safety, and healthcare topics.
Take on a leadership role in new employee orientation and field visits, as needed. About You :
Education : RN and BSN required (or a minimum of 5 years in a healthcare setting); Baccalaureate degree preferred.
Experience : At least 1 year in home care or community health settings and experience with frail or elderly populations preferred.
Licensure : Active, unrestricted RN license in New York State.
Technical Skills : Proficiency in computer systems and managed long-term care insurance experience is a plus. Your Skills & Abilities :
