Aliados Health (AH) is a network comprised of a 17-member community health centers with sites in Contra Costa, Marin, Napa, Solano, Sonoma, and Yolo counties. Formed in 1994, our mission is to improve access to, and the quality of care provided, for underserved and uninsured people. AH’s major focus is to strengthen the capacity of member health centers to act as a regional network of comprehensive primary care that provides access to consistently high-quality clinical care through support in Policy, Populations Health, Health Informatics, and Access and Equity.
Additionally, AH serves as a HRSA designated Health Center Controlled Network, which serves an expanded region in Northern California across a 13county region : Marin, Mendocino, Napa, Nevada, Placer, Santa Clara, San Francisco, Shasta, Sierra, Siskiyou, Solano, Sonoma, and Yolo through projects in Health Information Technology, Analytics and Health Equity.
POSITION SUMMARY
Reporting to the Director of Transformation Initiatives at Aliados Health and stationed at a member health center, full-time in Marin County, CA with support and oversight from site staff at this location, the Community Health Worker is responsible for building rapport with patients and providing culturally responsive care to support patients in accessing a variety of health and social service needs.
The Community Health Worker will be responsible for screening patients for healthcare and social service needs (including needs regarding routine healthcare visits), meeting with patients based on an established schedule, providing health navigation and health education services, and documenting encounters in the patient’s electronic medical record.
The successful candidate will be excellent at connecting with members of the community, compassionate and patient, outgoing, possess a self- starting work ethic, and a strong commitment to community health and well-being of every member of our community, including, specifically, those within immigrant and newcomer populations. Ideally, the candidate is from or has a strong knowledge of the community they are serving.
ESSENTIAL DUTIES :
- Meet 1 : 1 with patients to screen for health and social service needs, identify gaps in healthcare, and connect patients with the appropriate resources
- Collaborate with the clinical care team to discuss patient needs and coordinate support
- Provide health education on topics related to lactation, pediatric care, preventative care, and other subjects to be determined
- Help reduce linguistic and cultural barriers to care
- Provide support for newcomers about services relevant to their needs
- Guide patients through processes of accessing health and social services, providing follow up as needed.
- Help patients with registration paperwork and social service applications as needed.
- Document interactions with patients in their Electronic Medical Record
- Meet with the care team, site supervisors, and Aliados Health supervisor on a regular basis, TBD
- Participate in CHW networking peer groups
- May perform other duties as assigned from time to time.
