Job Description

We're a home health company looking for a dedicated, compassionate home health Case Manager to grow their career with our family!

Here’s what you’ll do :

conduct initial and ongoing comprehensive assessments of home health patients
oversee OASIS documentation at regular intervals
implement / develop / document the plan of care to ensure quality and continuity of care
provide care utilizing infection control measures that protect both the staff and the patient (OSHA)
supervise the home health aide and LPN team to ensure the highest quality of service
assure continuity of quality patient care delivered with appropriate documentation
perform on-call responsibilities and provide on-call service to patients / families, as assigned
provide effective communication to patient / family, team members, and other health care professionals

Candidate must :

be an actively licensed Registered Nurse (RN)

have at least one year of case management experience in home health care

submit to a criminal background check, DMV check, and reference check

have a driver’s license, insurance, and reliable transportation
