What are the responsibilities and job description for the Disability and Aging Case Manager/Intake Coordinator position at Align Services?
Case Managers will monitor, coordinate, and evaluate participants’ plans of care; advocate for participants with disabilities and the aging, and ensure participants’ safety and well-being during services while complying with Medicaid and Wyoming Department of Health rules and regulations. Assist participants and their families with the application and eligibility process. Case Managers will adhere to Align Services values.
Job Responsibilities
- As the intake coordinator, you will serve as the primary contact for those applying for the Community Choice Waiver and assisting them through the application and eligibility process, then assigning the appropriate case manager.
- As a case manager, you will serve as the primary contact for guardians, families, and providers of participants on your caseload
- Communicate and collaborate with parents, guardians, providers, and others in the participants’ circle of support to ensure participants’ wants and needs are being met
- Solve problems effectively and facilitate problem solving with the participants’ team
- Monitor participants’ progress toward goals; modify and implement new goals as needed
- Maintain accurate and up-to-date documentation of participants’ services and activities according to state and Medicaid rules and regulations; submit reports within required timeframes
- Develop a plan of care for each assigned participant on the Developmental Disabilities and the Community Choice waiver, detailing the type of support needed in each area of their lives
- Monitor participants’ services to ensure their plan of care is being followed by providers
- Lead team meetings to monitor services and address unmet needs or concerns
- Provide targeted case management services for participants on the Developmental Disabilities Waiver waitlist and assist them in navigating the application and eligibility process
- Monitor providers’ documentation and billing information to ensure adherence to Medicaid rules and regulations
- Manage Individual Budgeted Amounts (IBAs) of participants for support services, in amounts upwards of $200,000 per year
- Research and connect participants with community resources (i.e. various therapies, Food Stamps, Cheyenne Housing Assistance, Employment Support, etc)
- Monitor participants’ overall health and medical care, ensuring follow up is completed as needed
Required Skills
- Effective and professional communication skills – written and verbal
- Knowledge of basic computer software (MS office, Adobe, etc.) and standard office equipment (printer, scanner, copier)
- Excellent grammar, spelling, punctuation
- Basic and some algebraic math skills required
- Detail-oriented
- Must be able to follow instructions, take initiative, be able to organize and prioritize tasks
- Effective time management
- Critical thinking skills and the ability to problem solve effectively
Other Requirements
- Maintain CPR/First Aid certification
- Complete and pass a background check
- Certification as a case manager through the Wyoming Department of Health
- Obtain 8 hours (min) of continuing education each year
- Maintain up to date driver’s license and vehicle insurance
- Attend all Align Services and Division trainings within required time frames
Education
- Master’s degree from an accredited college or university in one (1) of the following related human service fields:
- Counseling
- Education
- Gerontology
- Human Services
- Nursing
- Psychology
- Rehabilitation
- Social Work
- Sociology
- A related degree as approved by the Wyoming Department of Health
OR
- Bachelor’s degree in one of the above-mentioned fields AND one (1) year of work experience as a case manager or in a related human services field
OR
- Associate’s degree in one of the above-mentioned fields AND four (4) years of work experience as a case manager or in a related human services field
Job Type: Full-time
Pay: $16.00 - $21.00 per hour
