Position Summary:
This job description reflects management’s assignment of essential functions. It does not prescribe or restrict the tasks that may be assigned. Nothing in this job description restricts management’s right to assign or reassign duties and responsibilities to this job at any time.
Responsibilities:
- Meets with patients to research discharge planning needs as presented by the treatment team.
- Meets one-on-one with patients from admission until discharge at a minimum of one time per week to discuss case management.
- May be assigned to conduct educational lecture or group activity.
- Communicates with Business Development Department and referring provider to coordinate appropriate aftercare resources.
- Under the guidance of the Clinical Director, reviews and researches aftercare resources.
- Supports treatment team and patients in communication with referral and continuing care resources.
- Documents continuing care plans, communications and meetings.
- Prepares and distributes copies of patient schedules weekly as requested.
- Attends treatment team meetings as assigned.
Qualifications
Education:
- Bachelor’s Degree in Behavioral Science or related field preferred.
- High School Diploma/GED required
Experience:
One years experience working in a behavioral health care facility preferred.
