The Role Summary:
Will assist in providing comprehensive case management services to the eligible population who struggle with mental health, substance use, or co-occurring disorders, as well as provide support for their families and significant others. Will also make necessary referrals and coordinate services, by promoting linkages between mental health and/or substance treatment services, primary care clinics and other social service providers to ensure that clients have access to all available and necessary services to obtain an optimum level of functioning. Make frequent contact with clients in their natural settings and/or where clients are receiving care, focus on practical problems of daily living, promote assertive advocacy, and assistance with transportation if needed.
Required Education and Experience:
- Bachelor’s degree in Sociology, Psychology, Social Work, or Human Services.
- Two years of related experience.
