Customized Community Supports

CCS specialists are responsible for implementing a person-centered program which includes skills training in identified areas of need to include activities of daily living, adult educational supports, citizenship skills, communication, social skills, self-advocacy, informed choice, cultural exposure, community integration and relationship building opportunities with the objective of encouraging growth of natural supports and community independence.

• Able to provide transportation for program participants in your own vehicle,
• Must have good organizational and communication skills,
• Maintaining current first aid and CPR certification and all DDSD required training's,
• Assures the health and safety of Individuals while working in the community,
• Implements behavior support plan & supports Individuals with behavioral challenges, including but not limited to physical restraint,
• Must be able to lift 50lbs or more as moderate physical exertion required, including but not limited to bending, sitting, standing, squatting, kneeling, running, walking, climbing stairs, lifting, transferring, participating in sport activities, etc.,
• Able to work directly with individuals supported including but not limited to challenging behaviors, physical restraint, medical care, personal care, grooming, hygiene, assisting with medication, lifting and transferring through the entirety of the shift,
• Ability to work alone with individuals supported who are or can become verbally and/or physically aggressive on a daily basis and support them through crisis events while implementing behavior support plan,
• Ability to hear and withstand repetitive noises, sounds, phrases, yelling, screaming, etc.

Customized In-Home Supports

Customized In-Home Supports (CIHS) are intermittent services and/or supports that are individually designed to instruct or enhance home living and community skills and to address health and safety as needed. CIHS provides people the opportunity to design and manage the services and/or supports needed to live in their own home or their family home.

CIHS include a combination of instruction and personal support activities provided intermittently when they would normally occur to assist the individual with activities of daily living (ADL), health related supports, meal preparation, household services and money management. Supports also include providing support to acquire, maintain or improve interaction skills in the community or at the person’s place of employment.
