Summary
As a Registered Nurse - Home Health at All America Home Healthcare, you will play a vital role in delivering personalized nursing care to seniors in their homes. This position is essential in supporting our mission of providing compassionate healthcare while ensuring the well-being and dignity of our clients.
Responsibilities
- Conduct comprehensive assessments of patients' health status and needs.
- Develop and implement individualized care plans based on patient assessments.
- Monitor vital signs and document medical information accurately.
- Provide specialized care for patients with dementia and other chronic conditions.
- Administer medications and manage pain effectively for patients.
- Educate patients and their families about health management and care plans.
- Collaborate with interdisciplinary teams to ensure coordinated patient care.
- Maintain accurate medical documentation in compliance with healthcare regulations.
Qualifications
- Current Registered Nurse (RN) license in the state of Indiana.
- Proven experience in home health or similar nursing roles is preferred.
- Strong skills in medical documentation and patient assessment.
- Experience in dementia care and pain management is highly desirable.
- Ability to monitor vital signs accurately and respond to changes in patient conditions.
- Familiarity with phlebotomy practices is a plus but not mandatory.
- Excellent communication skills for effective interaction with patients and families.
- Compassionate approach towards elderly care, demonstrating respect and dignity.
