POSITION OVERVIEW
The Clinic RN uses an integrative approach to promote wellness and address the medical, and social determinants of health of an assigned panel of patients. This position coordinates with the care team and other community resources and supports to optimize outcomes.
SCHEDULE AND PAY
This is a full-time (40 hour) position.
Clinic Open: Monday – Thursday 8AM - 7PM, and Friday 8AM – 5PM Scheduling needs will be discussed during interview (Monday-Friday).
- Non BSN pay range starts at $34/hour - $36/hour.
- BSN pay range starts at $36/hour -$38/hour.
DUTIES
- Delivers quality, comprehensive nursing care to patients at ACHC as part of a comprehensive team, including but not limited to;
- Triage, rooming of patients, dispensing medication, providing education, utilizing effective nursing communication and motivational interviewing skills, refilling medication
- Facilitates the integration between the medical, dental, and social services departments
- Coordinates and leads daily team huddle meetings
- Facilitates health services for patients who are being referred from outside ACHC
- Ensures the provider team’s schedules are at capacity
- Provides care coordination services and evidence based interventions that improve health outcomes
- Supports medication management focusing on treatment adherence, medication refills and monitoring any possible side effects
- Facilitates transport of patients to the hospital when a higher level of care is needed
- Follows up on referrals and facilitates referrals to outside facilities as requested by provider staff
- Assists with primary and secondary prevention efforts including education, screening etc. as indicated.
- Develops population management strategies and interventions to assist ACHC in meeting established metrics for the medical and mental health
QUALIFICATIONS
- Bilingual Spanish Desired
- Registered Nurse (RN) required
- Bachelor’s degree in Nursing from an accredited nursing program preferred
