POSITION OVERVIEW

Responsible for providing comprehensive counseling and support services to individuals struggling with substance use disorders.

SCHEDULE AND PAY

This is a full-time (40 hour) position. Clinic Hours are Monday Thursday 8AM - 7PM, Friday 8AM 5PM. Scheduling needs will be discussed during interview. Salary range : $18- $21 an hour, based on education and experience.

DUTIES

Assists with providing patient care that addresses social determinants of health.

Assists with conducting assessments for social determinants of health (SDoH).

Provides direct services or connects patients to programs to address SDoHs.

Educates patients on available community resources and how to access them.

Assists with providing case management to referred patients.

Conducts assessment of patients current social, financial, and emotional needs.

Assesses available resources and connects patients to programs.

Assists patients with formal and informal referrals

Assists with applications for medication, transportation, housing and other SDoHs

Assists patients with phone calls.

Facilitates group sessions with patients.

Conducts outreach and networking as necessary.

Coordinates referrals with community organizations.

Maintains and builds relationships with community resource agencies

Assists in maintaining and updating All Care Health Centers Community Resource List.

Participates in outreach events as requested by supervisor.

Maintains documentation for all consultation notes, significant incident occurrences, phone conversations, etc.

Completes documentation within 24 hours

Maintains documentation consistent with applicable regulatory requirements.

Other duties as assigned.

QUALIFICATIONS

Must possess a high school diploma or General Educational Development (GED) certificate.

A minimum of one year of experience providing case management to individuals, families, and groups preferred.

Must have working knowledge of county / state benefit programs, the health care delivery system, local health care providers and community resources.
