POSITION OVERVIEW
Responsible for performing a variety of social work services within commonly accepted standards of social work practice ranging from simple to crisis cases.
SCHEDULE AND PAY
This is a full-time (40 hour) position. Clinic Hours are Monday – Thursday 8AM - 7PM, Friday 8AM – 5PM. Scheduling needs will be discussed during interview. Salary range : $55,000-$60,000, depending on certification and experience.
DUTIES
Responsible for providing patient care that addresses social determinants of health.
Conducts initial assessments for social determinants of health (SDoH).
Provides direct services or connects patients to programs to address SDoH’s.
Assists individuals and / or families in finding satisfactory ways of coping with and understanding the nature and extent of problems in social situations and relationships.
Educates patients on available community resources and how to access them.
Serves as the main point of contact for patients who present with any type of abuse or neglect.
Responsible for providing case management to referred patients.
Conducts assessment of patients current social, financial, and emotional needs that are affecting a patient’s medical condition, treatment, recovery, or transition.
Assesses available resources and connects patients to programs.
Assists patients with formal and informal referrals
Assists with applications for medication, transportation, housing and other SDoHs
Assists patients with phone calls
Conducts outreach and networking as necessary.
Coordinates referrals with community organizations.
Maintains and builds relationships with community resource agencies
Maintains and updates All Care Health Center’s Community Resource List.
Participates in outreach events as requested by supervisor.
Maintains documentation for all consultation notes, significant incident occurrences, phone conversations, etc.
Completes documentation within 24 hours
Maintains documentation consistent with applicable regulatory requirements.
Other duties as assigned.
QUALIFICATIONS
Bachelor's degree in Social Work or similar field.
Possesses a current, valid certification as a Social Worker in the State of Iowa.
A minimum of two years of experience providing case management to individuals, families, and groups preferred.
Must have working knowledge of county / state benefit programs, the health care delivery system, local health care providers and community resources.
