Description
- To provide services in client's homes per Care Plan for each client, following safe practices to promote client's ability to maintain at home in a safe environment.
- Responsible for observing and practicing safe environment by following infection control policies.
- Responsible for the client's personal care and assistance of Activities of Dily Living (ADL), examples include bathing, shampooing, skin care, oral hygiene, dressing, toileting, transfers, etc.
- Responsible for essential homemaking related to safety and cleanliness of the client's environment, examples include meal prep/feeding, laundry, light housekeeping, errands, etc.
- Communicate effectively and accurately both orally and in writing with all members of the Agency team.
- Report changes in the client's condition and/or home immediately to supervisor or office.
- Attend scheduled Inservice meetings.
- Demonstrate safe practices in the use of equipment. Does not do anything not trained to do and notifies supervisor of training needs.
- Adheres to Standard and Universal Precautions.
- Maintains confidentiality in all aspects of position per Agency policies.
- All other duties as assigned.
Qualifications
- Minimum of high school diploma or GED
- Be at least 18 years old
- Prefer minimum of six months experience in the field or a facility
- Maintain current TB test and CPR
- Ability to interpret direction for care and proceed in carrying out written and verbal orders given by supervisor
- Ability to accurately report written and verbal information regarding client care
- Ability to effectively handle the physical demands of position on a daily basis
- Ability to be compassionate and understanding of client's and family needs
- Current driver's license and dependable transportation preferred.
- Minimum of current state identification
- Be able to pass required background checks
