As a Care Manager, you will be responsible for developing care plans, coordinating services, and providing support to both persons supported and their families. You will work closely with our team of caregivers to ensure that each persons supported receives the personalized care and attention they deserve.
Our ideal candidate is someone who is empathetic, organized, and detail-oriented. You must have excellent communication skills and the ability to work well under pressure. Previous experience in a healthcare or caregiving role is preferred, but not required.
