If you align with these values, this may be just the place for YOU!Salary : An estimated salary for this position is $60,000 - $70,000 per yearKey Responsibilities : Assisting the Qualified Professional department with client support services such as home visits, intakes, and evaluations.Assisting with adding new service for clients and coordinating with the QP to role out the new service.Advocating for clients with case managers, counties, and the State of Minnesota to ensure their needs are met.Reporting suspected abuse or maltreatment to state and county authorities as required.Supporting investigations into potential fraud involving caregivers or clients in coordination with HR and other departments.Completing documentation accurately and in a timely manner.Collaborating with other departments to address client safety concerns and sensitive issues.In this role, you'll need to be adaptable, handle various challenges, be a team player and engage with clients from diverse backgrounds across the Twin Cities area. A Master's degree is preferred.Current License Social Worker (LSW) through Minnesota Board of Social Work.1-2 years of experience in mental health advocacy, crisis intervention, or case management preferred.Knowledgeable with state and county systems, including compliance with Department of Human Services (DHS) guidelines.Skills and Abilities : Strong communication and documentation skills for preparing reports and coordinating with case managers, counties, and state agencies.Ability to assess and address client needs effectively during home visits, intakes, and evaluations.Proficiency in identifying and investigating potential fraud or abuse involving caregivers or clients.Problem-solving skills with the ability to exercise sound judgment in sensitive situations.Competent in using word processing, scheduling, and documentation software.Familiarity with electronic case management systems is a plus.Demonstrates reliability, tact, and the ability to work independently.Maintains confidentiality and adheres to ethical standards in all interactions.Capable of performing physical activities such as standing, walking, and lifting up to 50 pounds.Comfortable working in office settings with occasional outdoor visits to clients' homes.Additional RequirementsAbility to meet the background check standards as defined by Minnesota Statutes.Willingness to attend required training, including DHS workshops, within the first 90 days of employment.Performing these tasks according to DHS guidelines will ensure recipients of in-home care services receive appropriate care that meets their expectations. Being a team player and assisting other nurses with covering visits is also expected of all our nursing staff.
