Care Coordinator
All of Us In Recovery (A.I.R.) offers comprehensive case management services to help clients access essential community resources, including behavioral healthcare, peer support, housing, medical care, advocacy, education, employment, and social/spiritual opportunities to support their recovery journey.
Position Summary:
Care Coordinators are responsible for supporting individuals in accessing and navigating community resources to improve their well-being and achieve recovery goals. This includes working closely with clients to assess their needs, develop personalized care plans, and connect them to services such as healthcare, housing, employment, and social support. Care Coordinators collaborate with service providers, track client progress, and ensure ongoing support to promote successful community reintegration and reduce recidivism. Strong communication, documentation, and problem-solving skills are essential for this role. The position serves participants in the Free Through Recovery, Community Connect, and 1915i Medicaid Programs.
Required Qualifications:
- Bachelor’s degree in social work or related field OR 4 years of case management experience
- Valid driver’s license
- Basic data entry and documentation skills
- Goal-oriented, self-driven with a passion for advocacy
Preferred Qualifications:
- Completion of ND State-required training
- Experience working within the criminal justice system, including knowledge of the Parole and Probation process
Compensation:
Commission-based pay, dependent on the clients you work with. Part-time hours with the potential to transition to full-time as your caseload grows.
