What are the responsibilities and job description for the ACT MHP position at All Out Community Care Services?
POSITION SUMMARY:
Our Assertive Community Treatment (ACT) team is an evidence based practice that aims to improve outcomes for individuals who are diagnosed with severe and persistent mental illnesses and who are at a greater risk of psychiatric crises, hospitalization, and involvement in the criminal court system. Our ACT team is a group of professionals working to assert outreach in the community to assist our clients with improving their quality of life through consistent caring and person-centered services.
As a member of the ACT team, the Mental Health Professional (MHP) is qualified in a mental health profession, and possesses the clinical skills and experience to assess, plan, develop, coordinate and provide treatment, rehabilitation and support services to program individuals with severe and persistent mental illness under the clinical supervision of the team leader and psychiatrist.
POSITION DUTIES:
- Provide all services in a community-based setting
- Engages clients in established trusting, collaborative relationships directed toward the goal of symptom management and recovery
- Assist clients in identifying and developing their individualized/person centered goals of treatment
- Collaborate with natural collaterals utilizing psychoeducation techniques as needed
- Provide individual and group therapy, choosing applicable modalities according to individual needs and treatment plan
- Provide mini treatment team leadership
- Complete discipline specific assessments as outlined by program requirements
- Develop (with team assistance) comprehensive treatment plans per client
- Educate and support individual’s families and advocate for the individual’s rights and preferences.
- Assist team leader as needed or directed by team leader/program director
- Provide on call crisis intervention as required (on a rotation basis with stipend pay)
WORK SCHEDULE:
Monday – Friday 8:00 AM – 4:30 PM, with rotating weekend shifts (as required per team). On-call rotation with team members (~8-10 weeks)
Qualifications:
- Master’s degree in human services/related field; social work, counseling, psychology
- 1-year clinical experience working with adults with severe mental illness.
- LMSW/PLPC not required but highly preferred
- Knowledge and understanding of recovery principles and recovery driven service systems.
- Valid LA Driver’s License with no restrictions with state and federal agencies to practice in the State of Louisiana.
