Supervisor of Community-Based Services
Founded in 2001, All Pointe Care, LLC provides person-centered services to families, adults, young adults, teens and children. Services are designed to meet the individual goals and unique needs of each person or family. All Pointe Care aims to empower families and individuals to set and reach personal goals by fostering independence and inclusion and treating others with dignity and respect.
We are seeking an ambitious Supervisor of Community-Based Services to oversee various programs and initiatives who shares our mission of positively impacting the lives of individuals and families in Connecticut.
This position includes -
Program Development and Management : Develop, implement, grow and evaluate community service programs to ensure they meet the needs of individuals and families in the Department of Developmental Services’ supported programs.
Staff Supervision : Continuously build, lead, mentor and motivate a team of case managers and field staff, providing guidance and support to enhance their skills and performance.
Collaborations : Internal – Collaborate with the recruitment team to attract, hire and retain the best candidates for each position. External - Build partnerships and leverage relationships with government agencies and local organizations to foster a network of support and resources.
Compliance : Ensure the completion of annual assessments, support strategies, quarterly reports, and other documentation as required by regulations.
Event Oversight : Collaborate with teammates to develop and execute educational peer events and departmental initiatives within the budget
Qualifications and Benefits
- Strong track record of successfully growing and developing teams of case managers and field staff. This involves not only helping with recruiting and hiring the right individuals, but also providing ongoing training, mentorship, and support to ensure their professional development
- Bachelor’s degree in special education, human services or a closely related area or equivalent years of experience in the human services field
- Minimum of two-years practical experience working with individuals in the Department of Developmental Services’ supported programs
- CPR, PMT, first aid certification and state trainings are required (or willingness to obtain)
- A current and valid CT driver’s license.

Annual Pay : $60,000 - $70,000 Incentives
