Job Summary:
We are hiring a Social Worker to support our Behavioral Health Unit in Boise, ID. This is a full-time contract role for 3 months. You’ll work with patients, families, and healthcare teams to support care planning, discharge coordination, and emotional support services.
Key Responsibilities:
Help patients and families plan for discharge and next steps.
Complete assessments, forms, and reports.
Arrange transportation and follow-up care.
Attend interdisciplinary team meetings.
Provide information about Medicare/Medicaid and financial programs.
Support patients and staff with social and emotional concerns.
Maintain patient privacy and follow HIPAA rules.
Track and report grievances.
Participate in quality improvement efforts.
Qualifications:
At least 3 years of experience as a Social Worker.
Knowledge of healthcare discharge planning.
Strong communication and organizational skills.
Ability to work independently and with a team.
