What are the responsibilities and job description for the Licensed Vocational Nurse (LVN) position at All’s Well?
Qualifications:
Active California LVN or Licensed Psychiatric Technician license.
Graduate of a state-approved nursing program.
Minimum 1 year preferred in psychosocial rehab or working with disabled populations.
Active CPR/AED required.
Valid CA Class C driver’s license, insured vehicle, and insurability required.
Proficient in EHRs and Microsoft Office (Word, Excel, Outlook).
Knowledge of substance use disorders, dual diagnoses, and harm reduction practices.
Summary:
Provide direct patient care in a community-based mental health setting.
Work under the supervision of the Program Manager.
Deliver care that is patient-centered and recovery-focused.
Support the interdisciplinary team with physical health and medication management.
Address health issues and modifiable risk factors.
Help patients become active participants in improving their overall health and wellness.

Essential Duties and Responsibilities:
Provide support and coaching to members in areas like housing, employment, and community integration.
Administer, monitor, and document medications while ensuring proper usage and awareness of side effects.
Conduct health assessments and develop individualized care and recovery plans.
Monitor key health indicators (e.g., weight, BP, glucose) and assist with health education and disease management.
Collaborate with medical and interdisciplinary teams to address complex member needs.
Maintain accurate, timely documentation in the Welligent EHR system.
Conduct outreach visits and assist with transportation when necessary.
Participate in staff meetings and ensure compliance with program and county documentation standards.
