At our community-based mental health services agency, you'll be more than just a nurse – you'll be a vital partner in our members' lives. You'll provide direct, patient-driven care, addressing their physical health needs and medications with a focus on their unique goals and well-being.
Your Impact:
Empowerment: Coach, counsel, and advocate for members in housing, employment, community integration, and more.
Health Champion: Manage medications, monitor health conditions, and educate members on health promotion and disease management.
Collaborative Care: Work closely with psychiatrists, nurse practitioners, and an interdisciplinary team to provide holistic support.
Meaningful Connections: Build strong relationships with members, developing personalized Recovery Plans that truly make a difference.

What You'll Bring:
Current California LVN or LPT license.
A passion for patient-centered, recovery-based care.
Strong knowledge of medication management and physical health monitoring.
Excellent communication, organizational, and teamwork skills.
A valid driver's license, vehicle, and insurance.
