Job Summary:

Key Responsibilities:
- Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting.
- Provide mobility support, including transferring and repositioning patients safely.
- Monitor and report changes in patient health, behavior, or needs to the supervising nurse or healthcare professional.
- Assist with meal preparation, feeding, and ensuring dietary requirements are met.
- Perform light housekeeping duties, such as laundry and maintaining a clean living space.
- Provide companionship and emotional support to patients.
- Assist with medication reminders as per care plan guidelines.
- Follow infection control protocols and maintain a safe environment for patients.
- Transport or assist patients with mobility for medical appointments and errands, as needed.
Qualifications & Requirements:
- High school diploma or equivalent (preferred).
- Certification as a Patient Care Assistant, Home Health Aide (HHA), or Certified Nursing Assistant (CNA) is a plus.
- Prior experience in home care, assisted living, or healthcare setting is preferred.
- Ability to lift and move patients safely.
- Strong communication and interpersonal skills.
- Compassionate, patient, and dedicated to providing high-quality care.
- Ability to work independently and as part of a team.
- Must pass a background check and drug screening.
- Reliable transportation and a valid driver’s license (if applicable).
