What are the responsibilities and job description for the Nurse Coordinator position at Alleghany Highlands Community Services?
Alleghany Highlands Community Services provides services and support to individuals in our community with behavioral health challenges and intellectual and developmental disabilities. The Nurse Coordinator works as a team member providing person-centered active treatment and care for adults with developmental disabilities. The position requires coordination, implementation, documentation, and evaluation of health care services for nine individuals to assure complete integration with other services and the provision of active treatment.
REQUIREMENTS
- Associate's or Bachelor's Degree in Nursing (Required)
- 1 year experience in nursing field (Required)
- Registered Nurse with a current Virginia license (Required)
- Valid Driver's License (required)
RESPONSIBILITIES
- Provides comprehensive health care services to meet regulatory requirements, with the assistance of the consulting Medical Director and the team
- Complete pre-admission, post-admission and annual assessments of each resident's nursing needs
- Develops and directs nursing plan of care for each individual by coordinating with all service providers
- Provides training to AHCS employees including skilled nursing services and medication administration
