WHO IS Alleghenies United Cerebral Palsy (AUCP)- AUCP is a non-profit organization founded in 1955. It started as a grassroots effort by a group of concerned parents who wanted to find support for family members aged 18 – 59 with physical and intellectual disabilities. Starting in a church basement and growing into an organization that serves over 4,000 lives 18 and older, with staff located throughout the state of PA, our mission is to assist individuals with disabilities in meeting their basic needs, enhancing their quality of life, and promoting their independence, while also allowing them to be active participants in the decision-making and direction of their services.
***Due to company growth, we are looking for candidates to fill caseloads in the following Counties: Allegheny, Armstrong, Beaver, Greene, Indiana, and Washington.***
Job Responsibilities:
- SC meets with teams identified by clients and services to develop a plan that reflects the individual's priorities and how the team will assist with meeting those goals.
- SC updates the plan as needed to reflect what is currently happening in the individual's life and update services.
- SC coordinates and monitors services with paid providers and community resources to support the individual's needs and ensure they are meeting those needs, being utilized according to authorization, and that staff are knowledgeable and following the service plan.
- SC monitors individuals according to ODP regulations to ensure they are living Everyday Lives, living in the least restrictive environment, needing and receiving authorized services, and meeting all health and safety needs.
- SC communicates as needed with teams regarding updates, concerns, and issues and ensures they are addressed promptly and appropriately. Follows timelines and guidelines determined by ODP regulations and Alleghenies United Cerebral Palsy, including documenting and keeping up with billing requirements.
Requirements:
- SC must have a valid driver's license and an insured/registered vehicle.
- Educational Requirements: A Bachelor’s degree (B.A.) from a four-year college or university in a Social or Human Services or related field is required (i.e., social work, psychology, education, etc.).
- At least three (3) years of professional experience in case management
- Three (3) years of experience in working with individuals with Autism.
- Must pass an MA Fraud Check, PA State Police Background Check (e-PATCH), Child Abuse clearance, FBI check, Social Security Verification, and a DMV check.
Job Type: Full-time
Starting Hourly Rate: 19/hour, with the ability to get a raise to 20/hour after a probationary period and performance review.
